SUMMARY:ECHO-Chicago: Pain Management & Opioid Use Disorder in Older Adults Registration
DESCRIPTION:Adults over 65 years of age are at higher risk of experiencing chronic pain and adverse events from opioids and opioid use disorder (OUD). Older adults are more likely to inadequately report their pain due to age-related health\, which may lead to undertreatment of pain. In the last two decades\, opioid prescriptions to treat chronic pain among older adults increased at a rate of nine times what they were previously. This surge is accompanied with a drastic increase of older adults visiting emergency departments due to opioid abuse\, misuse\, overdose\, and addictions to heroin and cocaine. In consequence\, chronic pain\, opioids\, and OUD have become a major crisis in the United States among older adults.\nThe Opioid Use and Pain Management in Older Adults (I-COPE) program is an intervention that offers providers a set of smart tools for a more effective and efficient geriatric pain\, opioid\, and OUD management. The aim of the I-COPE program is to evaluate integration of shared decision-making\, patient-centered clinical decision support tools\, and ECHO model to address the critical need to integrate effective treatment for older adults with chronic pain\, opioid use\, and OUD. Patient-centered clinical decision support (PCCDS) tools provide clinicians with information presented at the right time and tailored to the individual patient\, improving communications\, care\, and patient-provider satisfaction. Shared decision-making (SDM) is a highly effective collaborative framework when there are many choices and there is uncertainty about the optimal treatment choice.\nECHO-Chicago in partnership with I-COPE Principal Investigators has developed an 8-week curriculum to offer more geriatric clinical content and EHR updates with real-word cases. Each session will include didactic topics delivered by an interdisciplinary team of content experts\, and two 15-minute problem-solving exercises using real-world cases brought by participating PCPs. PCPs will review how they can use the I-COPE Program toolkit with each case. DESCRIPTION:We ask for you to join us over two half days to: \n1) Re-engage on the meaning and currency of Black harm reduction.\n2) Identify ways to protect and uplift our own history of leading this work.\n3) Identify ways to ensure that Black people are centered in national and state changes in drug policy\, criminal legal\, family services\, and health care systems by promoting Black wellness as the primary goal. This event will be part of an ongoing series in which we will work to ensure that priorities can be aligned with local and regional issues. \nWe want to underscore that this meeting is\, like our 2018 convening\, a safe space for Black harm reductionists and Black stakeholders. We believe that we need that space to work on goals of improving Black community health and well-being to define the role(s) that a Black Harm Reduction Network can play in addressing the pernicious role of anti-Black racism in the nation’s policy response. \nAbout the Black Harm Reduction Network: \nEstablished in partnership with the Legal Action Center (LAC)\, the Drug Policy Alliance (DPA) and the National Harm Reduction Coalition (HRC)\, the Black Harm Reduction Network was created in 2013 to promote a national dialogue that 1) centers racial equity across the harm reduction framework\, including in drug policy\, and criminal justice; 2) elevates the history and current role of Black people in the movement; and 3) identifies and advances recommendations to reduce the harms of these intersecting systems on Black people and other marginalized groups. \nSince our inaugural meeting in 2017\, the term “Harm Reduction” has been so colonized that it is almost meaningless in the face of threats to the health and well-being of Black people in America. \n\n2 Day Event : Dec 9\, 2021 12:00 PM & Dec 14\, 2021 12:00 PM \nRegistration Link

What does this actually mean and why should anyone care? \nOpioids have been used for centuries for their pain relieving properties as well as their pleasurable effects. But more recently\, Opioid Use Disorder has affected many Americans and has led to the death of thousands. \nDr. Katherine Austman\, MD is an Addiction Medicine and Family Medicine Physician with Family Health Care of Gibson City\, Prairie Family Medicine & Obstetrics\, GAHHS. She specializes in substance use disorder\, family practice and maternity care. Dr. Austman became interested in Medical Addiction Treatment due to the growing number of patients that she was seeing with a co-occurring substance use disorder\, which was particularly concerning in her care for expectant mothers. \nIn this presentation\, Dr Austman will explore Opioid Use Disorder and its effect on our communities. The Opioid ECHO will also train clinicians who have the DATA-2000 waiver but who are not yet prescribing. Project ECHO (Extension for Community Healthcare Outcomes) is a hub and spoke educational model that connects specialists in academic medical centers (“hubs”) to primary care clinicians in rural and underserved communities (“spokes”) via videoconferencing. DESCRIPTION:The training aims to provide participants with the background knowledge and practical skills that they need to address suicidal risk and behaviors in clients in care for substance use disorder treatment. Participants will have the opportunity to increase their knowledge and apply practical skills in the following areas: \nApproaching Your Work: Learn how to manage reactions related to suicide and maintain a collaborative\, non-adversarial stance. Acquire the necessary skills to address potential conflicts between a care professional’s goal to prevent suicide and relapse and a client’s goal to eliminate psychological pain via suicidal behavior. \nUnderstanding Suicide: Gain an understanding of the definitions and language used when talking about suicide\, as well as the data that are relevant to addressing suicide in substance use disorder treatment including risk and protective factors\, warning signs\, and the complicating factors of substance misuse\, including opioids. \nGathering Information: Identify key points in treatment where a suicide assessment should occur\, what questions to ask to learn more about a client’s suicidal thoughts and behaviors past and present\, and how to ask them. Participants will practice asking questions in an interactive learning environment designed to help build confidence. The training presents key scenarios\, such as when to seek supervision or consultation and what to do when someone discloses suicidal thoughts during a group treatment session. \nFormulating Risk: Practice synthesizing assessment information into a risk formulation that will help inform next steps in treatment. AMSR emphasizes the importance of using a risk formulation not for prediction but as information to make a collaborative decision regarding recovery-oriented treatment planning. \nPlanning and Responding: Review suggested actions to take based on a risk formulation using resources from SAMHSA’s TIP 50 and evidence-based interventions.
